Authorize your insurance company or a licensed carrier to take possession of your vehicle. Use this form for total-loss claims, adjuster-directed moves, and carrier-to-carrier transfers.
Who uses this: Insurance adjusters, body shops, salvage yards, or any third party authorized by the registered owner.

Authorize a family member, friend, or other trusted person to pick up your vehicle on your behalf. The registered owner must complete and sign this form.
Who uses this: Registered owners who cannot pick up their vehicle in person and want to designate someone they trust.

Request access to retrieve personal belongings from a stored vehicle. Oregon law allows registered owners to collect personal property without paying towing or storage fees first.
Who uses this: Registered owners who need to retrieve items from a vehicle currently in our lot.

- Dispatch reviews all requests during business hours (Mon–Fri 8am–5pm).
- You will be contacted by phone or email to confirm pickup arrangements.
- Storage fees continue to accrue until the vehicle is released.
- Bring valid photo ID to the lot. The authorized person must match the name on the form.
